I have a request. I need some units SOP template, specially if you have one for helicopter formation flying. I want to write one, but I need a template to see, what should be adressed in one. Any suggestions are welcome.

Sorry I can't just send you what I have but I'm thinking my boss would have my arse if I just gave it to you.
For helicopter formation SOP, you obviously need to address the same issues as in a normal sortie brief (weather, notams, timings, nav, other considerations) but you also need to focus the entire flight (all helicopter crews) on the formation particulars. Things I'd mention (just off the top of my head) are minimum spacing (ie. no closer than 2 rotor diameters), speeds, callsigns, who squawks for the flight, white or red strobes, max angle of bank, max RoC and RoD, approach and departure considerations (ie. you don't want the rest of the aircraft hovering downwind of you if you're lead) and of course a HUGE focus on emergencies (normal aircraft emergencies...what will you and the others do; inadvertent IMC, lost contact, rejoin procedures).
Long and short of formation, if you don't do it often, start slow and wide spacing and work SLOWLY back in to your minimum spacing. Ensure you, and all the other participants, are qualified and are completely comfortable with what you're going to do. DO NOT go out in marginal weather!
